What Are the NMC Revalidation Forms?

Every nurse and midwife renewing their registration must complete and submit four forms as part of revalidation. You submit them to your confirmer (usually your line manager), who signs off before your renewal is sent to the NMC.

How to Fill Each Form: Step by Step

Form 2: CPD Record

List all CPD activities from your current revalidation cycle. The NMC requires:

  • 35 hours total — at least 20 hours must be participatory (with others, e.g., courses, workshops, peer discussions)
  • For each entry, record: date, activity title, hours, whether participatory, and how it relates to the NMC Code

Form 3: Practice Hours Record

Log the hours you've worked providing direct or indirect care. The NMC requires:

  • 450 hours for nurses registered on one part of the register
  • 900 hours for nurses registered on two parts (dual registrants)
  • Record date, setting, hours, and total running total

Form 6: Confirmation Form

Your confirmer completes this after your reflective discussion. It confirms that you've met all revalidation requirements. Common mistakes:

  • Don't fill this before the reflective discussion — it must be completed after
  • The confirmer must be NMC-registered and have worked with you for at least one period of practice
  • Ensure your confirmer has seen your portfolio before signing

Common Mistakes When Filling NMC Revalidation Forms

  • Submitting Word docs without checking formatting — download a fresh template each time; old versions may have expired NMC logos
  • Not linking CPD to the Code — every entry must show relevance to one of the four themes. Vague entries like "Updated my knowledge" don't meet the standard
  • Using the same reflective account for multiple entries — each account must be a different experience
  • Leaving practice hours to the last month — the NMC can audit your records going back three years
  • Waiting for your confirmer — book your reflective discussion at least 4-6 weeks before your renewal date
