The NMC provides four downloadable Word templates for revalidation. (Forms 1 and 5 are completed inside MyNMC online: there are no downloadable versions.)

How to Fill Each Form: Step by Step

Form 2: CPD Record

The NMC requires 35 hours total: at least 20 hours must be participatory. For each entry, record date, activity title, hours, type, and relevance to The Code. Not sure what counts? Read What Counts as CPD for NMC Revalidation?

You need 5 written reflective accounts. At least one must be about CPD or feedback, at least one about practice, and at least one linked to The Code. 5 Common Mistakes When Filling NMC Forms

  1. Using outdated templates: always download fresh copies. Old versions may have expired logos and formatting issues
  2. Not linking CPD to The Code: vague entries like "Updated my knowledge" don't meet the standard. Be specific about which Code theme
